The game's mechanics in the Ironman mode appear to be targeted towards experienced players who have a long time ago finished their end-game content and are looking for new challenges. This is not to say that the game aren't played and loved by everyone both old and new. I must take my bow to Jagex for this The mechanic operates in ways I didn't comprehend until I experienced it.
